--Your/the student's name and phone number (and whether it is home, cell, work, etc)--Your/the student's education/work situation (availability, middle school/high school/college, occupation, etc.)
--Dates and times you are available to meet (Be broad, but list your preference. Also, I give priority in scheduling to those with whom I meet more.)
--Whether you want me to come to you(address) or meet at the library(name)
--How you heard of my service
--How long you would like to meet for our first session--Number of hours/meetings per week (consistency is most efficient, at least twice weekly, as the tutoring work becomes undone if too much time passes)
--Any target dates I should be aware of
--Specific areas you need help with
--Information specific to you or your situation, such as prior grades in math classes and overall--If prior tutoring was unsuccessful, please explain
If you seek academic tutoring (for school or college), I will also need to know:
--School/college name
--Course title
--Where you are in the course
--Additional information on the class you have, such as:
-syllabus
-textbook(s) used (ISBN, or title, author, edition and publisher)
-what sorts of homework and exams there are, etc.
-information on the instructor
-Whether I have permission to contact your/your child's instructor. If
yes, please notify them that I will be contacting them, please provide
me their contact info, and understand that we agree that I have said
permission until you explicitly state otherwise.
If you are studying for the GMAT or GRE, I will also need to know:
--What book you are using (Title, author, edition, ISBN)
--How many years since you last dealt with math
--The last math course you took
--How much time you are able to allocate to studying (outside of meeting with me)
--The problem types with which you have the most difficulty
